Concussion Forms

The State of Wisconsin recently passed Wisconsin Act 172 which requires improved education of sports related head injuries and concussions for parents, athletes and coaches.  Educational fact sheets will be given to parents, athletes and coaches and Agreement Forms must be signed and completed before participation in practice or games.
